Licensed Practical Nurse (LPN) – All Shifts

River Ridge Living Center is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our growing nursing team. As an LPN, you will play a vital role in providing high-quality nursing care to residents in our skilled nursing and rehabilitation facility. The ideal candidate is committed to delivering exceptional resident-centered care while working collaboratively with physicians, registered nurses, and interdisciplinary team members.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor, assess, and document residents' health status and report changes to the supervising RN or physician.
• Provide direct nursing care and support to residents in accordance with individualized care plans.
• Assist with admissions, transfers, and discharges.
• Maintain accurate and timely clinical documentation.
• Supervise and support Certified Nursing Assistants (CNAs) in the delivery of resident care.
• Respond promptly to resident needs and emergencies.
• Ensure compliance with facility policies, infection control procedures, and state and federal regulations.
• Communicate effectively with residents, families, physicians, and healthcare team members.
• Participate in care planning and interdisciplinary team meetings.

Qualifications

Required:

• Current New York State Licensed Practical Nurse (LPN) license in good standing.
• Graduate of an accredited Practical Nursing program.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate, dependable, and resident-focused approach to care.

Preferred:

• Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or hospital setting.
• Experience with electronic medical records (EMR/EHR) systems.
• Knowledge of state and federal nursing home regulations.
