Job Description


We are seeking a licensed Physical Therapist to provide comprehensive patient evaluations, develop individualized treatment plans, and deliver evidence-based therapeutic interventions. The Physical Therapist will work collaboratively with interdisciplinary healthcare teams to improve patient mobility, reduce pain, restore function, and enhance overall quality of life. This role requires strong clinical assessment skills, patient-centered care, and proficiency in documentation and care coordination.


Key Responsibilities

  • Perform comprehensive patient evaluations and functional assessments to identify physical impairments, limitations, and rehabilitation goals.
  • Develop and implement individualized treatment plans based on clinical findings, physician referrals, and patient needs.
  • Administer therapeutic exercises, manual therapy, neuromuscular re-education, gait training, balance training, and physical therapy modalities.
  • Monitor patient progress, document outcomes, and modify treatment plans to maximize functional improvement.
  • Educate patients, families, and caregivers on home exercise programs, injury prevention, and ongoing rehabilitation strategies.
  • Maintain accurate, timely, and compliant documentation within electronic medical record (EMR/EHR) systems.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated patient care.
  • Ensure compliance with state licensing requirements, Medicare/Medicaid regulations, facility policies, and professional practice standards.
  • Participate in quality improvement initiatives, continuing education, and professional development activities.
  • Provide compassionate, patient-focused care while maintaining the highest standards of clinical excellence.


Required Qualifications

  • Bachelor's, Master's, or Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program.
  • Current and unrestricted Physical Therapist license in the state of practice.
  • Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
  • Experience developing and implementing individualized treatment plans.
  • Excellent communication, interpersonal, and patient education skills.
  • Proficiency with electronic medical records (EMR/EHR) and basic computer applications.
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team.
  • Commitment to delivering high-quality, patient-centered care.


Preferred Qualifications

  • Minimum of two (2) years of clinical experience in a healthcare, rehabilitation, outpatient, inpatient, home health, or skilled nursing setting.
  • Experience with outcome-based treatment planning and functional mobility assessments.


Schedule

  • Full-Time
  • 35 hours per week
  • Monday through Friday
  • No weekend rotation required
