Dear Patient Care Candidate

The DiaBuddy Experience — Powered by Caresync Concierge — is on a mission to put a smile back into healthcare by dispensing care, compassion, and empathy instead of prescriptions. As a Concierge Service Provider (CSP), you will serve as a certified DiaBuddy Service Provider (DSP), delivering remote, relationship-first care to patients living with pre-diabetes, diabetes, and other chronic conditions. Through our telehealth model, you will engage patients wherever they are — at home, on their schedule — offering lifestyle modification coaching, wellness education, and meaningful human connection.

This is not a traditional clinical role. We believe prevention is a cure for chronic conditions, and your work will reflect that philosophy every day. You will be part of a nationwide network of pharmacy students and healthcare professionals who have chosen a non-traditional, compassionate pathway in patient care. As a CSP, you have the opportunity to operate as a virtual health and wellness clinic, earning income based on the positive outcomes you create for your patients and community.

Responsibilities

• Conduct remote telehealth consultations with patients living with pre-diabetes, diabetes, and other chronic conditions to provide personalized lifestyle modification guidance.
• Build meaningful, ongoing patient relationships focused on engagement, education, and human connection rather than transactional prescription management.
• Develop and deliver individualized digital care plans, wellness solutions, and health and spiritual wellness recommendations to patients at home.
• Collaborate with and mentor pharmacy students and other healthcare professionals within the DiaBuddy network, upholding program standards and patient care excellence.
• Partner with the Diabetes Foundation and other non-profit organizations to advocate and provide care for underserved communities navigating chronic illness.
• Maintain accurate patient records and outcome tracking, leveraging digital databases and networking tools to optimize care delivery and program impact.
• Operate your independent virtual health and wellness clinic, managing your own schedule and growing your patient base to maximize positive health outcomes and income potential.

Minimum Qualifications

• Bachelor's degree in pharmacy, healthcare, or a related field.
• 3–5 years of relevant experience in patient care, health coaching, or a related discipline.
• Strong verbal and written communication skills for effective remote patient engagement.
• Comfort with digital care platforms, databases, and telehealth technology tools.
• Self-motivated and able to work independently with a flexible, self-directed schedule.
• Deep alignment with a patient-first philosophy centered on empathy, compassion, and prevention.
