As a Concierge Service Concierge (CSP) you will be able to dispense care, compassion, and empathy, along with over a hundred (100) digital care plans, instead of prescriptions. As a Concierge Service Provider (CSP), you will be able to dispense care, compassion and empathy along with over one hundred (100) digital care plans in our nationwide DiaBuddy Experience Virtual Wellness Practice. We are looking for individuals who want to transform their career as health care professionals, so our goal is to educate, empower, inspire and then hire health care professionals and others who want to deliver remote, relationship-first care to patients living with pre-diabetes, diabetes, and other chronic conditions. Through our telehealth model, you will engage patients wherever they are — at home, on their schedule — offering lifestyle modification coaching, wellness education, and meaningful human connection. This is not a traditional clinical role. We believe prevention is a cure for chronic conditions, and your work will reflect that philosophy every day. You will be part of a new nationwide network of healthcare professionals who have chosen a non-traditional, compassionate pathway in patient care. As a CSP, you have the opportunity to operate and own a virtual health and wellness clinic, earning income based on the positive outcomes you create for your patients and community.

Responsibilities

Conduct remote telehealth consultations with patients living with pre-diabetes, diabetes, and other chronic conditions to provide personalized lifestyle modification guidance and support.

Build meaningful, ongoing patient relationships focused on engagement, education, and human connection rather than transactional prescription management.

Develop and deliver individualized digital care plans, wellness solutions, and health and spiritual wellness recommendations to patients at home or in the community.

Collaborate with and mentor other healthcare professionals within our network, upholding program standards and patient care excellence.

Partner with the Diabetes Foundation and other non-profits plus faith-driven, cause-related organizations to advocate and raise awareness to the programs and services they provide for underserved communities navigating chronic illness.

Maintain accurate patient records and outcome tracking, leveraging digital databases and networking tools to optimize care delivery and program impact.

Operate your independent virtual health and wellness clinic, managing your own schedule and growing your patient base to maximize positive health outcomes and income potential.

Minimum Qualifications

Bachelor's degree in pharmacy, healthcare, or any related field at least two years of relevant experience in patient care, health coaching, or a related discipline.

Strong verbal and written communication skills for effective remote patient engagement and a burning desire to help others.

Comfort and familiarity with digital care platforms, databases, and telehealth technology tools.

Self-motivated culture and ability to work independently with a flexible, self-directed schedule.
